  2. Identification of the material that was removed or disabled and its location before removal.
  3. A statement under penalty of perjury that you have a good-faith belief the material was removed or disabled due to mistake or misidentification.
  4. A statement that you consent to the jurisdiction of the federal district court for your judicial district, or if outside the United States, that you consent to jurisdiction in any district where the service provider may be found.
  5. A statement that you will accept service of process from the person who filed the original DMCA notice or that person’s agent.
  6. Your physical or electronic signature.

If we receive a valid counter-notification, we may restore the material in accordance with applicable law unless the original complainant files an action seeking a court order.
